           University ID cards for Domestic Partners
 
As of September 1 domestic partners as well as spouses and dependent
family members of faculty, staff, and graduate students are eligible
for University ID card privileges.  The application form for the ID
card requires documentation that reflects sharing a household and a
relationship of financial interdependency.  There is a $5 fee for
each card.  Information and application forms may be obtained Monday
through Friday between 9:00 a.m.  and 4:30 p.m.  at the following
locations:
