There are two components to this cooperative agreement program. The control technology component will evaluate the effectiveness of engineering control or personal protective equipment in preventing occupational exposure to blood. Project evaluation will be based upon the efficacy of exposure prevention, cost analysis, and impact on patient care. Behavioral evaluation projects will assess the ability of interventions to affect organizational, social and/or individual health-care workers' behaviors. The goal is to improve compliance with CDC recommendations and to generate data upon which to base recommendations for practical methods of increasing worker compliance. Both components require development and maintenance of a data management system for the study; and development of communication models for informing management and labor of the nature of the work hazards and for modifying attitude and behavior. FUND NO: Announcement 529 DESCRIPTION: The Centers for Disease Control and Prevention (CDC) announces a program to conduct epidemiologic and behavior research studies of AIDS and HIV infection. These studies must evaluate the implementation and effectiveness of policies to reduce mother-to-child HIV transmission, examine factors related to mother-to-child HIV transmission, or evaluate factors associated with health, long-term HIV-seropositive persons. The study of minority populations is encouraged. Research on policies to reduce mother-to-child transmission should evaluate the implementation and effectiveness of guidelines for universal counseling and voluntary HIV testing of pregnant women; for offering zidovudine (ZDV) to HIV-infected pregnant women; and for providing needed health and social services for HIV-infected women and their children. Studies of mother-to-child transmission should identify HIV-infected women during pregnancy or at delivery and enroll the women and their infants in a prospective follow-up study to examine factors related to transmission, early diagnosis of infant infection, and disease progression. Studies examining the effect of interventions to prevent mother-to-child HIV transmission are of particular interest. Applicants must demonstrate that they can provide adequate rates of follow-up of both mothers and infants. Each year, at least 30 HIV-positive mother-infant pairs must be enrolled and followed at each study site. The third type of study, prospective evaluations of healthy, long-term HIV-seropositive persons, should assess virologic and host factors in which the HIV-infected persons remain disease-free or asymptomatic for prolonged periods (usually 10 yrs. or more). Preference will be given to studies in which cohorts of long-term health HIV-seropositive persons with characterized dates of seroconversion and high CD4+ lymphocyte counts (greater than 500 cells/mm3) have been established. Cooperative agreement applicants will address these research objectives in a collaborative manner with CDC. All applicants must be willing to participate in collaborative studies with other CDC-sponsored projects, and must specify whether they are applying for a project grant or cooperative agreement. For the purposes of this program, out-of-home child care is defined as care provided to children outside the home for at least ten hours per week in child care centers, family child care homes, family group homes, or similar settings. The primary population of interest is children five years of age and younger and their families; however, children up to thirteen years of age (and their families) attending after-school care programs may be included in the study population. Providers of health care services should be the focus for recruitment and data collection. Projects should quantitatively assess infectious disease morbidity (both in the child and in the child's family) associated with out-of-home child care; and associations between morbidity and the type of health care utilized by children and other family members. Health care provider-focused interventions having a measurable impact on morbidity among children and their families should also be assessed. Grantees should initiate and complete surveillance for infectious disease morbidity; definition of the impact of common respiratory illnesses, respiratory complications, and related antibiotic use on morbidity among children, family members, and child care providers; or assessment of the effectiveness of health education and its impact on antimicrobial use and resistance. FUND NO: PA-95-072 DESCRIPTION: The National Institute on Drug Abuse announces the availability of funds to stimulate international research on similarities and variations in drug abuse behaviors; factors influencing the initiation, progression, and cessation of drug abuse; and social and health consequences of drug abuse including HIV transmission. Increased understanding of the role of social, cultural, political and economic factors on drugs abuse behaviors is sought. Comparative studies with U.S. populations are encouraged. International research is encouraged in order to provide both a broader context for the examination of social and cultural variables and the opportunity to assess these variables in differing environments. Research may also investigate mediating and protective factors and processes in populations with very low drug use. These results could help to develop effective preventive methods that could be transferred or adapted to populations where drug abuse is more prevalent. Applicants should describe how the project will further the field of drug abuse epidemiology through the use of unique resources, populations, or environmental conditions in other countries and explain how the project may yield knowledge that will benefit the United States. Studies may address, for example, the influence of social, cultural, political, and economic factors on the relationship between drug use and HIV transmission. FUND NO: PA-95-070 DESCRIPTION: The National Institute of Mental Health (NIMH) announces the availability of funds to support research to identify brief interventions that can be implemented in clinics, other health care facilities, and community-based organizations to prevent the spread of HIV. Top priority will be given to interventions accessing hard-to-reach populations at high risk for HIV. This program announcement solicits theoretically grounded research applications focusing on developing models of brief HIV prevention programs. When designing proposals, two problems should be taken into consideration. First, HIV education campaigns primarily providing information about the HIV virus, modes of infection, and methods of prevention do not produce sustained behavior change. Second, models of brief interventions accessing hard-to-reach populations that may not otherwise be exposed to HIV prevention programs are urgently needed. Areas of interest include: 1) the most effective interventions in different settings; 2) the best setting for brief interventions, and the best intervenor; 3) critical components that will motivate individuals to assess HIV risk and seek help; 4) brief, effective counseling models; 5) interventions most likely to produce sustained behavior change; 6) possible barriers to conducting interventions and how they can be overcome; 7) appropriate means of defining, operationalizing, and measuring the effectiveness of a given intervention; 8) expected outcomes; 9) realistic behavioral outcome goals for different individuals and groups; 10) factors mediating test-seeking behavior; and 11) possibility of setting standards for the content and process of prevention interventions to be delivered in various settings.
